Department of Education Access Card Request Form

Graduate students, Faculty and Staff may request an Access Card that will allow after-hours access to the Department and to select facilities within the Department.

* New cards, replacement cards and clearance updates to existing cards can be requested by completing the request form.
* Studnet card access expires automatically on May 31st and December 31st. Extend/reactivation requests can be made using this form.
* Processing of requests can take up to a week.
* You will be notified by email when your card is ready to be picked up.
